Step 1: Verify we accept the patient's insurance here 

Step 2: Choose a Location for retina exam

Dallas, Gun Barrel City, Mt. Pleasant, or Greenville, TX.

Select "New Patient" and choose date and time.

Step 3:  Please fax or e-mail the following:

1) The insurance card (front and back) or indicate self-pay, 

2) photo ID, and 3) last eye exam note.
